Two training organisations offering the "Green Card" safety course have been suspended for six months for violating exam rules.
Industry groups have slammed the misconduct, saying it undermines construction site safety.
The Mandatory Basic Safety Training Course -- commonly known as the "Green Card Course" -- is required for anyone entering construction sites, and aims to raise workers' safety awareness.
There are currently over 130 training providers accredited to offer the course.
The Labour Department found that one provider had disclosed all examination questions and answers to trainees before the exam.
The trainer also allowed students who did not meet attendance requirements to take the test.
In another case, no invigilators were present during the exam, allowing candidates to discuss answers.
The green cards granted to the trainees involved have been withdrawn and cancelled.
